2018 MS BOCO REGISTRATION

October 12-14 | Middle Schoolers!

We are headed back to Camp Timberline in Estes Park for BOCO October 12-14th. We will be joining our friends from Cornerstone and First Pres. for an awesome weekend experience. Register today to claim your spot.

**Unfortunately, transportation will NOT be provided to and from camp. Drop-off and Pick-up will be required from the family. It is about a 1 hr. drive from BVCC. We will do our best to coordinate with volunteers to create a car-pool.

Drop off is Friday, Oct. 12th at 6pm.
Pick up is Sunday, Oct. 14th at 1pm.

PACKING LIST:
-Sleeping Bag
-Pillow
-2 set of clothes that can get dirty!
-Jeans and/or sweatpants
-One Plain White T-shirt that will be ruined
-Other T-shirts/Sweater/Sweatshirt
-Dark clothing for night games
-Closed-Toed Shoes for Mountain Adventures
-Socks
-Underwear
-Toiletries
-Towel & washcloth
-Shower shoes
-Warm pajamas
-Heavy jacket
-Hat, gloves/mittens
-Rain jacket or poncho
-Pen, Bible, Journal
-Water Bottle
-Flashlight
-$30 Spending money (optional – for snacks, souvenirs, etc.)

-Theme Dinner “Totally Rad 80's Night”: come dressed in some fun neon and look like your parents for Friday nights dinner!

Things NOT to bring:

-Electronic (iPods, cell phones, gaming devices, etc.)

Health Form/Liability Release

In consideration of being accepted by Boulder Valley Christian Church for participation in Roots Student Ministry Events, we (I), being 21 years of age or older, do for ourselves (myself) (and for and on behalf of my child-participant if said child is not 21 years of age or older) do hereby release, forever discharge and agree to hold harmless Boulder Valley Christian Church and the directors thereof from any and all liability, claims or demands for personal injury, sickness, or death, as well as property damage and expenses, of any nature whatsoever which may be incurred by the undersigned and the child-participant that occur while said child is participating in the above-described trip or activities.

Furthermore, we (I) [and on behalf of our (my) child-participant if under the age of 21 years] hereby assume all risk of personal injury, sickness, death, damage and expense as a result of participation in recreation and work activities involved therein.

Further, authorization and permission is hereby given to said church to furnish any necessary transportation, food, and lodging for this participant.

The undersigned further hereby agree(s) to hold harmless and indemnify said church, its directors, employees, and agents, for any liability sustained by said church as the result of negligent, willful, or intentional acts of said participant, including expenses incurred attendant thereto.

(If the participant has not attained the age of 21 years):

We (I) are (am) or legal guardian(s) of this participant, and hereby grant our (my) permission for him (her) to participate fully in said trip, and hereby give our (my) permission to take said participant to a doctor or hospital and hereby authorize medical treatment, including but not in limitation to emergency surgery or medical treatment, and assume the full responsibility of all medical bills, if any.

Further, should it be necessary for the participant to return home due to medical reasons, disciplinary action, or otherwise, we (I) hereby assume all transportation costs. Consent and release is ( __ is not) hereby given to display video and or pictures of the participant.
